Background: Patient safety is an essential component in improving the quality of health services. In primary health care facilities, the implementation of risk management and patient safety programs plays a crucial role in preventing medical errors and improving service quality. However, the implementation of patient safety in primary health care facilities is still not optimal in some areas.. Method: This study used a descriptive quantitative approach. The sample consisted of 50 health workers working in primary health care facilities selected using purposive sampling. Data were collected using questionnaires and observation checklists related to risk management and patient safety implementation. Data were analyzed using descriptive statistics in the form of frequency distributions and percentages.. Result: The results showed that 72% of health workers had good understanding of patient safety principles, while 20% were categorized as moderate and 8% as low. The implementation of risk management activities such as incident reporting reached 65%, patient identification procedures were implemented by 80% of respondents, and adherence to standard operating procedures reached 70%. Conclusion : The implementation of risk management and patient safety in primary health care facilities is generally good, but improvements are still needed in incident reporting systems and staff training to enhance patient safety culture.
